Turning Back the Clock: The Four Stages of Care

November 06, 2026•3 min read

There’s no single treatment that takes years off. But there is a plan. Here are the four stages we use to help patients look and feel like themselves again.

“Can you make me look like I did ten years ago?” I hear this question often, and I love it, because it tells me exactly what a patient is hoping for: not a new face, but their own face, rested and bright again.

The honest answer is that no single syringe can do it. The face ages in layers, over years. So we rebuild it the same way: in stages, each one setting up the next. This is a framework, not a promise of a set number of years, and every plan is personal. Here’s how the journey usually unfolds.

Stage 1 · Remediate: address the accumulated damage

First, we take care of what time has already done. That usually means three things: softening expression lines, replacing lost volume, and repairing skin texture.

This stage builds the foundation. Contouring on top of deep lines, hollow areas or dull skin never looks as good as contouring on a refreshed base.

Stage 2 · Accentuate: refine with facial contouring

Once the base is restored, we refine. Facial contouring uses filler to bring back balance and definition where structure has softened: the cheekbones, the jawline, the chin and the lips.

The goal is proportion. Lifting the cheek a few millimeters can soften the folds around the mouth. Supporting the chin can sharpen the jawline. Small, well-placed changes make the whole face look more like you. (For a deeper look, read Facial Contouring, Explained.)

“We’re not adding features. We’re bringing yours back into focus.”

Stage 3 · Optimize: support your body from within

This is where Pure Med Spa is different. How your skin heals, holds volume and responds to treatment is tied to what’s happening inside your body.

For many women, perimenopause and menopause bring changes in energy, sleep, weight and skin all at once. With lab-guided bioidentical hormone therapy, we look for balance, not just “normal” numbers. We also talk about metabolic health and weight, because both affect how the face ages, and how it changes as weight comes off. Ask us during your consultation which options make sense for you.

Stage 4 · Maintain: keep improving over time

Results last longest when they’re maintained, not started over. Once your plan is in place, we set a simple calendar so you always know what’s next. A typical rhythm looks like this, though your own timing may differ:

  • Every 3–4 months: a Botox, Dysport or Daxxify refresh
  • About once a year: a filler review and touch-up, area by area
  • Every 1–2 years: Sculptra, as collagen naturally declines
  • Periodically: regenerative facials to keep skin quality improving
  • Ongoing: hormone follow-ups and lab checks, as your plan needs

What a first year can look like

Every plan is different, but here’s an example of how a first year might come together for a patient in her late 40s:

  • Month 1: consultation, labs if appropriate, and a first neuromodulator treatment
  • Months 1–3: a Sculptra series to begin rebuilding collagen
  • Months 2–4: a series of PRF microneedling sessions for texture
  • Month 4: cheek and chin contouring once the base is restored
  • Ongoing: hormone therapy if it’s right for you, and a maintenance calendar

Where to begin

You don’t have to know which stage you’re in. That’s what the consultation is for. We’ll look at your face, your skin and your health, and map out a sequence that fits your goals, timeline and budget. To understand the thinking behind every plan, read Naturally You: My Philosophy of Care.
